Rowers involved in or witnessing an on-water occurrence is asked to complete an occurrence report form. This form was created in conjunction with the Coal Harbour stakeholders group. A copy of this form can be downloaded from the link below, as well as it can be found in the RED binder above the sign-out sheets in the shell bay. All incidents are to be reported and include:

• Collision/allision with any other marine vessel, floatplane
• Collision/allision with dock/marine structures (bad dockings not included)
• Near misses with above
• Report dangerous behaviour by other harbour users (reckless boating, endangering others, drinking, etc.)
• Noxious fumes and/or fuel spills

• When an incident occurs fill out the form IMMEDIATELY so that all-pertinent details are recalled. To the best of your ability please fill out all available information including all parties involved (and their contacts), as well as list of any witnesses.
• Notify Vice-Captain Operations and place completed form in the RED binder for further processing by the Vice-Captain Operations.
